State Accessibility Audit Log Template: A Comprehensive Model for Documenting Compliance
The State Accessibility Audit Log Template is a free tool that Georgia state agencies can use to plan, record, and report their accessibility audits. The template is designed for manual testing. Each page in a website is tested by hand with assistive technologies and methods, such as:
- screen readers
- screen magnifiers
- speech recognition software
- keyboard-only navigation
- contrast checks
Every barrier found is documented in the audit log.
As government agencies work to reach and maintain ADA Title II compliance, the audit log provides a consistent way to document what was tested, what was found, and what has been fixed.
Using the Template
The template is intended to be used by accessibility professionals, such as an agency's digital accessibility specialist or the accessibility auditors they work with. When evaluating a third-party vendor to conduct an audit, this template can serve as a model to ensure the auditor will be capturing all the necessary information.
To use the template, a new copy should be started for each audit. Each copy records:
- The scope of the audit
- The pages and screens selected for testing, and the reason each was chosen
- The browsers, assistive technologies, and other tools used for testing
- A result for every WCAG 2.1 Level A and AA success criterion, from a built-in checklist with a brief test procedure for each
- Each barrier found, with steps to reproduce, supporting evidence, the impact on users and a recommended fix
- The retest of each fix as the agency's team resolves findings, and its outcome
A Record of the Work
Over time, the completed workbooks become the agency's internal record of its accessibility work. They show which sites and pages were tested, the methods used, what failed, and which fixes were verified. Above all, they show that an agency or department team did the work itself, testing by hand, confirming each issue and verifying each fix instead of relying on third-party automated results alone.
From Audit Log to Report
The Summary sheet tallies findings and checklist results automatically as testing progresses. By the time testing is complete, the figures for an executive summary are already in place. The findings, with their descriptions and recommended fixes, supply most of the content for a full accessibility report.
Works With Existing Tools
Findings are stored one per row in a standard Excel table. Agencies that track work in an issue tracker such as Jira or Azure DevOps can export findings into their existing workflow and bring updates back into the log.
The template is also structured to accept results from automated accessibility checkers. Scanned pages and flagged issues give the audit a starting point, and each issue is confirmed by an accessibility professional before it is logged as a finding.
